2. Click the GREEN BUTTON - START NOW.
3. Is the person an essential worker -NO
4. Scroll down and click - CONTINUE
5. Does the person have symptoms -NO
6. Has the person been asked for a follow up test - NO
7. Is the person who needs a test part of a trial - None of the ABOVE
   
8. Why are you asking for a test? - I’ve been in contact with someone who’s tested positive for coronavirus
   
9. Contact tracers told me to get a test
10. Do you know your contact tracing code - YES
